REMINDER: USE SAME NAME IN ALL EXAMINATION FORMS. IF THERE IS AN ERROR IN SPELLING AND OTHER DATA KINDLY REQUEST YOUR

ROOM WATCHERS TO CORRECT IT ON THE FIRST DAY OF EXAMINATION. REPORT TO YOUR ROOM ON OR BEFORE 6:30 A.M. LATE EXAMINEES WILL NOT BE ADMITTED.

Examinees who reside/live or are currently located in Pampanga are required to undergo RT-PCR testing 3-5 days, and present the results of which on the day of the examination. However, in the event that they cannot comply with the said requirement, they may also present a 14-day quarantine certificate, dated October 5-18, 2021, signed by their respective RHU physician (with a dry seal and letterhead).

Examinees who live/reside outside Pampanga are required to submit negative RT-PCR test results, taken 3-5 days prior, on the day of the examination. They may also opt for saliva RT-PCR tests, provided that it is the Philippine Red Cross that administers the said tests, being the only FDA and DOH recognized provider of the said service. Although not encouraged, we will also acknowledge antigen tests taken 48 hours before the examination.

On top of these, examinees shall also present Annex A (on the first day) and Annex B (first and second

day). Furthermore, all examinees are required to present their SPASS.PH registration (on their mobile device).

These requirements are mandatory and failure to present either of the two, the examinee shall not be

allowed to take the licensure examinations.
